Monday - Friday No same day appointments.
Saturday - 8:00 am - Noon (Walk in only, must arrive by 11:45 to register)

A signed physician's order
Your insurance card
An insurance referral from your primary care physician
- if required by your insurance company
- if you have an electronic referral, please check that your primary care physician's
  office has submitted it


Preparing for Your Visit

To avoid changing into a gown, please wear comfortable clothes with no metal, e.g., no belts, buckles, snaps, zippers, or metallic decorations
